Has anyone ever tried forming 204R brass from 222 mag cases?

Never tried, but the 222mag is the parent case of the 204R. I would think that all you would have to do is run the 222 cases through a 204R full length die. Just go slow and use Imperial case sizing wax, and lube the necks. Check for proper length, trim if needed, and you will want to anneal the brass. This is the process that we use to form our brass for the 20VT from 221 fireball brass.
If you have 222 mag brass available, and are in need of 204R brass, this might be something you want to try.

That's what I thought but I checked with my local gun shop and they said 222 mag brass is almost impossible to find. I guess I should have checked with them first before I posted my question.

If you had 222 Mag brass, you could likely trade it for 204 Ruger brass...there should be plently of 204 brass out there now, I think in the first couple years of the cartridge many Mag cases were converted due to the lack of brass and/or the popularity of the cartridge.
